Athletic Paperwork

 

Before the start of practice you must have your completed Athletic Eligibility packet submitted through Permission Click. All parts of the packet must be completed and signed including the doctor's physical. Paper forms are no longer accepted and all players must be on the eligible list before they can attend ANY practices or tryouts.
